High School Immunization Clinic for Meningitis

The Region of Peel has informed the school that almost half of our high school students do not have the proper immunization for meningitis and that a letter has been sent to these students. The Region has offered to run a free meningococcal immunization clinic at the Main Campus on Wednesday, March 25th. Parents should note that the Region does not require parental permission for your child to receive the immunization BUT the school will require parental permission. In order for your child to participate, please print off this form and return it to the Mentor or TEAM office. Please note that the Region has informed the school that this vaccination will be mandatory before the start of the 2015-2016 school year, that suspensions will be put in place if students are not vaccinated and that getting this immunization done privately could be costly as it is not covered by OHIP.
